The main reason the Vader is burned i lava is because right after the original came out in 1977, George set up this whole senario where Obi-Wan and Anakin fought and then fell into a lava pit and was disfigured. So, this idea bewcame cannon for the series back in 1977. I remember reading this in an official Star Wars magazine or something. It probably also shows up in some of the novelizations from before the prequals. Bottom line, old news, old idea that was cannonized long before this movie, and to change it would have screwed up the whole continuity of the franchise.
Oh, BTW, Palpatine let Windu get the upper hand on purpose and for a reason. The final corruption of Anakin. I read that on StarWars.com.
